I'm removing my Form Lighting LED headlights due to a medical issue. I have had no issues with them and they are in like new condition. They have been on my 20 TRD Sport for approximately a year. They will ship in the factory box with everything they came with when new. Shipping is on your dime, but will use whatever method you like. I'll be removing them from the truck this weekend. I'll post photos after removal.
